Diabetes doesn’t just affect the eyes, kidneys and heart — it can silently and insidiously disrupt the entire gastrointestinal (GI) tract, from the oesophagus to the colon. Even the pancreas and gallbladder aren’t spared. Diabetic gastroenteropathy is a complex, under-recognised complication driven by autonomic neuropathy, structural remodelling and altered gut–brain signalling. Its symptoms including nausea, bloating, diarrhoea, constipation, reflux and incontinence are often dismissed or misattributed, yet they impact quality of life, nutrition, drug absorption and can disrupt glycaemic management. By identifying people who are at-risk early, and optimising glucose, nutrition and medication choices, we can protect the entire gut–brain axis, sparing our clients the lifelong burden of a burdensome and progressive complication.
