Microvascular complications of diabetes embody the complex interplay of metabolic, inflammatory and genetic factors that contribute to the development and progression of these conditions. The entities which are clinically recognised most are diabetes related retinopathy, nephropathy, and sensorimotor neuropathies.
Management of microvascular complications in diabetes requires a multifaceted approach focusing on optimizing management of blood glucose, blood pressure and lipid levels on a foundation of healthy lifestyle changes. Although these strategies can delay the progression of complications, they are often insufficient to completely prevent or reverse tissue damage.
People with diabetes can exhibit heterogeneous responses to treatments. While some people respond well to intensive glycaemic management, others continue to exhibit disease progression despite optimal management. This variability in outcome may be attributed to genetic differences, variations in disease duration and the presence of other comorbid conditions. The era of personalized care in diabetes medicine holds much promise within this clinical domain such that the narrative of truly individualized care becomes the standard approach in the years ahead!
