The gut, often referred to as the ‘second brain’, has been shown to play a central role in the regulation of metabolic health. Emerging evidence suggests that gut health is intricately linked to the pathophysiology of diabetes as well as being implicated in the development and progression of diabetes-related complications. Our final Journal Club of 2024 will highlight the mechanistic interplay between the gut microbiota, inflammation, glycaemic management and the potential long-term complications associated with diabetes. The relationship between gut health and diabetes complications is bidirectional - poor gut health can exacerbate glycaemic dysregulation and inflammation, and diabetes-related hyperglycaemia can further impair gut function. Hyperglycaemia negatively impacts gut permeability and microbiota composition, creating a vicious cycle of worsening metabolic control and increasing risk for complications. Understanding this relationship may open new avenues for therapeutic interventions. Focus will be placed on foregut symptoms which include diabetic gastroparesis, gastroesophageal reflux disease (GERD), oesophageal dysmotility and increased risk of infections such as Helicobacter pylori. Together or individually these clinical conditions have significant implications for glycaemic management, and equally they can impair the quality of life in people with diabetes. Early recognition of and targeted management for such conditions are essential to mitigate symptoms and improve outcomes in people with diabetes.
